'It said that - contrary to decades of public health advice - switching from saturated fats found in foods like butter, cheese and fatty meats, to polyunsaturated fats such as vegetable oils and fish - did not seem to have any benefit for the heart. 
This surprised not only me but the people who co-funded the research, the British Heart Foundation (BHF). 
Pretty much every respectable health body says that we should cut down on food that is high in saturated fat because it can cause cholesterol levels in the blood to build up. 
Raised cholesterol increases your risk of heart disease. Some unsaturated fats can lower blood cholesterol so the assumption has been that this will cut your heart disease risk. 
But the analysis of dozens of international studies did not yield clear evidence that switching to mono and polyunsaturated fats reduced the risk of cardiovascular disease. ' says Fergus Walsh.  More...

“At least 33,000 lives could be saved each year if the nation ate more healthily,” reported theDaily Mail. “Eating five portions of fruit and vegetables a day would by itself prevent 15,000 early deaths, including 7,000 from coronary heart disease, almost 5,000 from cancer and more than 3,000 from stroke,” the newspaper continued.
 
The story is based on research that modelled what would happen if the average diet in the UK followed the recommendations for food consumption. In 2007, none of the UK countries met these recommendations. The study supports previous findings that good diet, together with other measures such as regular exercise and stopping smoking, cuts the risk of chronic disease and increases lifespan.
As the authors note, the modelling technique has some flaws and involves considerable estimation and assumptions, which may have led to an overestimate of the number of lives that would be saved if healthy eating guidelines were met. However, despite these flaws and even though the study cannot predict how diet influences risk for individuals, it does indicate that keeping to dietary recommendations reduces the risk of disease.

Where did the story come from?

The study was carried out by researchers from the University of Oxford. Funding was provided by the British Heart Foundation and the National Heart Forum. It was published in the peer-reviewed Journal of Epidemiology and Community Health.
The study was covered accurately in the media. The BBC and the Daily Mail both included comments from independent experts.

What kind of research was this?

This study used a computer model, called the Dietron model, to measure systematically the health impact of dietary changes at a population level. While research has already shown that diet is associated with the risk of cardiovascular disease and cancer, the researchers point out that such a model is needed to estimate accurately the effects of measures taken to improve diet in the population. This model linked the consumption of specific dietary components with disease risk factors (for example blood pressure, cholesterol and obesity) and subsequent mortality from coronary heart disease, stroke and cancer.
Modelling studies such as this always involve a considerable degree of estimation of the effect that an intervention (in this case dietary change) will have on the overall level of disease in a population. The links between diet and the risk of disease were taken from research reviews that had pooled the results of different studies in this area. The individual studies pooled in the reviews are likely to have been in a variety of populations, and used different methods, dietary interventions and follow-up assessments. In addition, the individual studies may not all have taken into consideration other potential confounding factors, particularly lifestyle factors such as smoking and exercise. Therefore, the risk associations that were put into the model and then used to link diet with actual diseases (coronary heart disease, stroke and cancer) may involve some degree of inaccuracy.

What did the research involve?

The researchers needed to obtain several sets of data to fill their model. Data for UK deaths from coronary heart disease, stroke and cancer were obtained from the Office for National Statistics, the General Register Office for Scotland and the Northern Ireland Statistics and Research Agency. Information on the population’s intake of foods and nutrients was obtained from two sources: the average intake of fatty acids, fibre and fruit and vegetables for 2005–7 was derived from the Expenditure and Food Survey, while estimates of salt intake came from the National Diet and Nutrition Survey, 2006.
The modelling also incorporated several meta-analyses of individual studies looking at diet and disease risk factors. The researchers looked at reviews that had pooled data from randomised trials, cohort studies or case-control studies, giving priority to meta-analyses of randomised trials. These different studies were combined in the model to calculate the change in risk of disease for an individual who changes his or her diet. To estimate the change in health outcomes with a change in diet at a population level, the model used the difference between current average consumption levels and recommended levels of different foods in the UK.

What were the basic results?

In a general summary of the main findings, the researchers calculated that:
  • About 33,000 deaths a year would be avoided if UK dietary recommendations were met
  • There would be a reduction in deaths from coronary heart disease of 20,800 (95% credible interval 17,845 to 24,069), a reduction of 5,876 for deaths from stroke (3,856 to 7,364) and a reduction of 6,481 for deaths from cancer (4,487 to 8,353).
  • About 12,500 of these avoided deaths would be in people aged 75 or under.
  • About 18,000 of the avoided deaths would be men and 15,000 would be women.
  • More than 15,000 of the avoided deaths (nearly half the total figure) would be due to increased consumption of fruit and vegetables.
  • Reducing average salt intake to 6g a day would avoid 7,500 deaths annually.
  • The greatest number of deaths avoided would be in Northern Ireland and Scotland, whose populations are furthest from achieving dietary recommendations.

How did the researchers interpret the results?

The researchers say their study suggests that increasing average consumption of fruits and vegetables to five portions a day is the target likely to offer most benefit in terms of deaths avoided. They also say that reducing recommended salt levels to 3g daily and saturated fat to 3% of total energy would achieve a similar reduction in mortality.
They conclude that their calculations based on the Dietron model are robust, pointing out that their estimate of deaths avoided is lower than a previous government survey which calculated that 70,000 deaths a year could be avoided if government dietary recommendations were met. The estimates could be used in calculating the allocation of resources for interventions aimed at reducing chronic disease.

Conclusion

This well-conducted modelling study used various data sources to link consumption of different dietary components with disease risk factors (for example blood pressure, cholesterol and obesity) and subsequent mortality from coronary heart disease, stroke and cancer. The study supports previous research showing that diet plays a crucial role in health and that a diet with plenty of fruit and vegetables, fibre and low fat and salt levels can reduce the risk of chronic disease, in particular coronary heart disease. However, its predictions are made at the population level. A model such as this cannot predict individual risk, which will depend on many factors, including family history, smoking and other lifestyle habits.
It is important to note that the figures are based on the estimates and assumptions made when using a mathematical model, and not on reality. As the authors themselves note, the modelling technique they used may have led to “some degree of double counting” and that, therefore, their estimate of reduced mortalities if dietary recommendations were met is likely to be an overestimate. Also, the accuracy of the model depends to some extent on the quality of the meta-analyses that were included, and the quality of the individual studies that were pooled within these reviews in order to establish associations between diet and particular disease risk factors.
Overall, this study supports current dietary recommendations and even though it cannot predict how diet influences risk for individuals, it does indicate that keeping to dietary recommendations reduces the risk of disease.
Dietary recommendations include eating five portions of fruit and vegetables a day (about 440g) and 18g of fibre (provided by wholegrain foods and some fruit and vegetables). It is recommended that salt intake is limited to a maximum of 6g a day and that a third of total energy is provided by fats, with saturated fat comprising 10%. The researchers point out that in 2007, according to the estimated average intakes in the sources they used, none of the UK countries met these recommendations.

Processed carbohydrates may increase health risks more than saturated fat does

Processed carbohydrates, which many Americans eat today in place of saturated fat, may increase the risk of obesity, diabetes and heart disease more than fat does.

In March 2010 the American Journal of Clinical Nutrition published a meta-analysis—combines data from several studies—compared the reported daily food intake of nearly 350,000 people against their risk of de-veloping cardiovascular disease over a period of five to 23 years. The analysis, overseen by Ronald M. Krauss, director of atherosclerosis research at the Children’s Hospital Oakland Research Institute, found no association between the amount of saturated fat consumed and the risk of heart disease.

Although saturated fat boosts blood levels of “bad” LDL cholesterol, it also increases “good” HDL cholesterol.

Saturated fats are not so bad; they indicate that carbohydrates could be worse.

A 1997 study Krauss co-authored in the Journal of the American Medical Association evaluated 65,000 women and found that the quintile of women who ate the most easily digestible and readily absorbed carbohydrates—that is, those with the highest glycemic index—were 47 percent more likely to acquire type 2 diabetes than those in the quintile with the lowest average glycemic-index score. (The amount of fat the women ate did not affect diabetes risk.) And a 2007 Dutch study of 15,000 women published in the Journal of the American College of Cardiology found that women who were overweight and in the quartile that consumed meals with the highest average glycemic load, a metric that incorporates portion size, were 79 percent more likely to develop coronary vascular disease than overweight women in the lowest quartile.

These trends may be explained in part by the yo-yo effects that high glycemic-index carbohydrates have on blood glucose, which can stimulate fat production and inflammation, increase overall caloric intake and lower insulin sensitivity, says David Ludwig, director of the obesity program at Children’s Hospital Boston.

Right now, Post explains, the agency’s main message to Americans is to limit overall calorie intake, irrespective of the source.

“We’re finding that messages to consumers need to be short and simple and to the point,” he says. 

Nobody is advocating that people start gorging themselves on saturated fats, tempting as that may sound. Some monounsaturated and polyunsaturated fats, such as those found in fish and olive oil, can protect against heart disease. What is more, some high-fiber carbohydrates are unquestionably good for the body. But saturated fats may ultimately be neutral compared with processed carbs and sugars such as those found in cereals, breads, pasta and cookies.

“If you reduce saturated fat and replace it with high glycemic-index carbohydrates, you may not only not get benefits—you might actually produce harm,” Ludwig argues. The next time you eat a piece of buttered toast, he says, consider that “butter is actually the more healthful component.”

Animal fats pancreas cancer link

Eating a diet high in red meat and dairy products is linked to an increased risk of pancreatic cancer, a US study has suggested.
Researchers followed 500,000 people who had completed a food diary for an average of six years.
The Journal of the National Cancer Institute paper found those who had the most animal fats in their diet had a higher risk of developing the cancer.
UK experts said cutting down on the fats was a way of reducing risk.
There has previously been confusion over whether there was a link between animal fats and pancreatic cancer, with different studies reaching opposite conclusions.
About 7,000 people are diagnosed with pancreatic cancer in the UK each year, with smoking being the biggest risk factor.
The prognosis is poor - the time between diagnosis and death is usually about six months.
'Welcome addition'
This latest research was carried out by the National Cancer Institute in Bethesda, Maryland, which felt earlier studies had been too small to give reliable results.
The participants were being followed to see if they developed a range of diseases.

Of the half a million studied, 1,337 developed pancreatic cancer.
Men who consumed the highest amount of total fats had a 53% higher relative rate of pancreatic cancer compared with men who ate the least.
In women, there was a 23% higher rate of the disease in those eating the most fat compared with those who ate the least.
Overall, people who consumed high amounts of saturated fats had 36% higher relative rates of pancreatic cancer compared with those who consumed low amounts.
Writing in the Journal of the National Cancer Institute, the researchers led by Dr Rachel Stolzenberg-Solomon, said: "We observed positive associations between pancreatic cancer and intakes of total, saturated, and monounsaturated fat overall, particularly from red meat and dairy food sources.
"We did not observe any consistent association with polyunsaturated or fat from plant food sources.
"Altogether, these results suggest a role for animal fat in pancreatic carcinogenesis."
In an editorial in the journal, Dr Brian Wolpin, of the Dana Farber Cancer Institute in Boston, and Dr Meir Stampfer, of the Harvard School of Public Health, said the study was a "welcome addition to the understanding of a disease that is in great need of new insights".
Josephine Querido, senior science information officer for Cancer Research UK, said: "This large study adds to the evidence that pancreatic cancer is more common in people who eat too much fat, particularly saturated fat.
"Understanding ways of reducing the risk of pancreatic cancer is very important because it can be very difficult to treat.
"Apart from stopping smoking, the best way to reduce your risk of cancer is to eat plenty of fruit vegetables and fibre, and to cut down on fatty foods, red and processed meat and limit your intake of alcohol."

Eating too much saturated fat can increase blood cholesterol levels, which in turn is a risk factor for coronary heart disease.

The healthier type of fat to choose is unsaturated fat (monounsaturated or polyunsaturated), which can actually improve cholesterol levels. But don’t forget all fat is high in calories, so if you are watching your weight, you should limit your overall fat intake.

Here are our tips to help you cut down on saturated fats:

  • Not so good fats…

    Butter, lard, palm oil, coconut oil/cream and ghee (clarified butter) are all rich sources of saturated fat and are best limited. Replace with small amounts of unsaturated fats such as olive oil, rapeseed oil or corn oil.

  • Choose your meat wisely

    Have chicken or turkey without skin, lean cuts of meats and trim off any visible fat. Cut back on fatty meat pies, sausages, sausage rolls and breaded meat or chicken etc. Did you know a grilled chicken breast without skin contains a third less saturated fat than with skin?

  • SpreadsMargarine on knife

    Use spreads thinly and go for lower fat spreads with the lowest amount of saturates. Avoid butter or hard margarines and choose spreads that mainly contain monounsaturated or polyunsaturated fats.

  • Check the labels

    Cut down on takeaways, ready meals and processed foods. Opt for lower fat versions; check and compare nutrition labels and pick foods with a lower amount of saturated fat.

  • Look at your cooking methods

    Grill, bake, steam, boil or poach foods instead of frying in lots of oil. Did you know a portion of cod fried in batter has 2.9g of saturated fat and 445 calories, but an average portion of baked cod has just 0.4g of saturated fat and only 115 calories! Rather than adding extra fat like cream or sour cream to recipes why not swap with low-fat yoghurt or fromage frais.

  • Dressings and sauces

    Make your own low fat salad dressing using ingredients such as low fat yoghurt, lemon juice, herbs, etc rather than shop brought ones. If you do buy dressing or sauces, pick lower fat varieties, e.g. low-fat salad dressing or lower fat mayonnaise.

  • Milk carton
  • Milk it up…

    Use semi-skimmed or skimmed milk rather than full fat or condensed milk. For example, swap a 200ml glass of full fat milk for semi-skimmed milk and cut your saturated fat intake by 2.9g and 40 calories.

  • To measure or spray

    Measure your oil out with a tablespoon rather than just pouring straight out of the bottle. Then you know how much you are using. Or why not use a spray oil to cook.

  • Cheesy choice


    Check out the nutrition label and opt for cheeses that are lower in fat and saturated fat such as cottage cheese, ricotta and half-fat cheddar. Did you know that a 30g portion of hard cheese is the size of a small matchbox?

  • Fish with lemon
  • Eat more oily fish

    Oily fish, such as salmon, mackerel, fresh tuna and sardines are rich in omega 3 polyunsaturated fat; which can help to protect against heart disease.

  • Snack attack

    For a tasty and nutritious snack, have a handful of unsalted nuts, seeds or dried fruit rather then reaching for fatty snacks such as crisps, pastries, or bombay mix. For those with a sweet tooth choose sugar free jelly or sorbets instead of cakes, biscuits or ice-cream.

  • Bulk up

    Add less meat to casseroles, stews and sauces and replace with fibre-rich vegetables, beans or pulses.
