PHE publishes new advice on vitamin D. Should we all take 10 micrograms per day Vit D all year?

Public Health England (PHE) is advising today (21st July 2016) that 10 micrograms of vitamin D are needed daily to help keep healthy bones, teeth and muscles.

This advice is based on the recommendations of the Scientific Advisory Committee on Nutrition (SACN) following its review of the evidence on vitamin D and health.

Vitamin D is made in the skin by the action of sunlight and this is the main source of vitamin D for most people. SACN could not say how much vitamin D is made in the skin through exposure to sunlight, so it is therefore recommending a daily dietary intake of 10 micrograms.  PHE advises that in spring and summer, the majority of the population get enough vitamin D through sunlight on the skin and a healthy, balanced diet. During autumn and winter, everyone will need to rely on dietary sources of vitamin D. Since it is difficult for people to meet the 10 microgram recommendation from consuming foods naturally containing or fortified with vitamin D, people should consider taking a daily supplement containing 10 micrograms of vitamin D in autumn and winter.

People whose skin has little or no exposure to the sun, like those in institutions such as care homes, or who always cover their skin when outside, risk vitamin D deficiency and need to take a supplement throughout the year.

Vitamin D regulates the amount of calcium and phosphate in the body, both needed for healthy bones, teeth and muscles. It is found naturally in a small number of foods including oily fish, red meat, liver and egg yolks and in fortified food like breakfast cereals and fat spreads.

Background 

The latest data from the PHE National Diet and Nutrition Survey (2008 to 2012) shows that 23% of adults aged 19 to 64 years, 21% of adults aged 65 years and above and 22% of children aged 11 to 18 years have low levels of vitamin D in their blood. This is not the same as having a deficiency, where you would be unwell, but rather means that you are at greater risk of developing a deficiency. If a person is deficient of vitamin D they will be clinically unwell and will need to be treated by a doctor.

PHE recommends against people using sunbeds because extreme short-term use could cause severe burning and long-term damage to the skin, with a possible increased risk of developing skin cancer.

SACN reviewed the evidence on vitamin D and health outcomes. In addition to musculoskeletal health, SACN reviewed the relationship between vitamin D and non-musculoskeletal health outcomes including cancer, Type 1 diabetes, multiple sclerosis and heart disease but found insufficient evidence to draw any firm conclusions.

The recommendations refer to average intake over a period of time, such as one
week, and take account of day-to-day variations in vitamin D intake. Vitamin D plays an important role in the regulation of calcium and phosphate in the body. It is therefore essential for bone health. Without adequate vitamin D, bones can become thin, brittle and mis-shapen.  In extreme cases this can lead to rickets in children, a condition involving a softening of the bones that can lead to fractures and deformity. In adults softening of the bones is called osteomalacia, and may cause pain and muscle weakness.

S.25 Mean dietary intakes of vitamin D from all sources (including supplements) were: 2-4 µg/d (80-160 IU/d) for ages 1.5-64y; 5 µg/d (200 IU/d) for adults aged = 65y.

S.28 For all age groups in the UK, mean plasma 25(OH)D concentration was lowest in winter and highest in summer. Around 30-40% of the population had a plasma 25(OH)D concentration < 25 nmol/L in winter compared to 2-13% in the summer. A large proportion of some population groups did not achieve a plasma/serum 25(OH)D concentration = 25 nmol/L in summer (17% of adults in Scotland; 16% of adults in London; 53% of women of South Asian ethnic origin in Southern England; and 29% of pregnant women in NW London).

Metabolism S.5 Vitamin D is converted to its active metabolite, 1,25-dihydroxyvitamin D (1,25(OH)2D), in two hydroxylation steps. The first hydroxylation is in the liver, where vitamin D is converted to 25- hydroxyvitamin D (25(OH)D), which is the major circulating metabolite of vitamin D and is widely used as a biomarker of vitamin D status; the second hydroxylation is in the kidney where 25(OH)D is converted to 1,25(OH)2D.

The proportion of the population (by age) with a plasma 25(OH)D concentration < 25 nmol/L was: 2-8%, 5m-3y; 12-16%, 4-10y; 20-24%, 11-18y; 22-24%, 19-64y; 17-24%, = 65y and above.

S.7 At latitudes below 37 degrees North, UVB radiation is sufficient for year round vitamin D synthesis. At higher latitude, vitamin D is not synthesised during the winter months. In the UK, sunlight-induced vitamin D synthesis is only effective between late March/early April and September and not from October onwards throughout the winter months.

My dentist takes a daily baby aspirin & Vitamin D

My dentist has taken a daily baby aspirin for 20-25 years because of research showing reduction in colon cancer (his father died of colan cancer). Would that research be from CPS II data which demonstrated a decreased risk of colon cancer in people who take aspirin regularly?...

The National Institute for Health and Clinical Excellence (NICE) has published new guidelines to help the NHS, local authorities and other organisations in their work to prevent skin cancer.
The guidelines say that some sun exposure is important, as it allows people to make vitamin D as well as providing an opportunity to be physically active.
But too much exposure to UV light is known to increase the risk of skin cancer.
Professor Mike Kelly, director of the institute's Centre for Public Health Excellence, revealed that about 100,000 people each year are diagnosed with non-melanoma skin cancer and over 10,000 with malignant melanoma, the most serious type of skin cancer.
He explained: "Through this guidance we hope to raise awareness of the risks of UV exposure and help people to protect themselves and others.
"Simple actions can greatly reduce the risk of developing skin cancer - opting to stay in the shade, wearing protective clothing in the sun, avoiding too much sun during the middle of the day and using sunscreen can all have an effect."
The guidance contains simple and practical recommendations for preventing over-exposure to the sun.
For instance, developers should attempt to create shaded areas around buildings, while schools should encourage children to use sunscreen and stay in the shade during breaks.

Sara Hiom, director of health information at Cancer Research UK, said that avoiding sunburn is one of the best ways to reduce the risk of skin cancer.
"Many of us like to make the most of the UK's rare sunny days and should be able to enjoy the sun safely.
"If we all make sure that our skin doesn't redden or burn in the sun, it could help to reduce the rocketing number of people who develop skin cancer every year. And that's the aim of Cancer Research UK's annual SunSmart campaign."

Vitamin D recommendations by Kurzweil/Grossman in Transcend

Lab Test Vitamin D - 25 hydroxy vitamin D 25(OH)D - normal reference range for lab they use is 32-100 ng/dL 

25(OH)D K & G recommend 50ng/dl. Daily multiple is 400 International Units (IU). If    < 20 25(OH)D take 5000IU/day, if 21-30 take 2000 IU/day, if 31-40 take 1000IU/day. Get lab test done after 3 months then use 1000-2000IU/day to maintain levels with lab tests ongoing to prevent excess levels.

Try to expose skin to <30mins per day direct sun before 10am or after 4pm.

 Note: According to UK Consensus statement (December 2010):-  Levels of Vitamin D below 25nmol/L (10ng/ml) (or 1000ng/dl) are “deficient”. People at risk of low sun exposure should take a 10 microgram supplement of vitamin D a day which is the Government recommended dose. NB. Boots General A-Z has 5 microgram Vitamin D per capsule per day (100% RDA).

Joint position statement issued to provide vitamin D clarity

Several newspapers have reported on new advice for sun exposure and vitamin D. Vitamin D is necessary to absorb calcium and form healthy bones. However, too much sun also raises the risk of skin cancer. Several large UK health organisations have made a joint statement about how much sun exposure would boost health without putting people at risk of its damaging effects.
The statement does not specify exactly how long people should aim to be in the sun for. This is because the time required to make sufficient vitamin D varies according to a number of environmental, physical and personal factors and may vary between individuals. The authors say that the time required is “typically short and less than the time needed to redden or burn”. Regularly going outside for a few minutes around the middle of the day without sunscreen is suggested as best and that “the more skin that is exposed the greater the chance of producing sufficient vitamin D before burning”. Importantly, this advice applies in the UK, and not necessarily in hotter climates.
Newspapers report that in practice this means between “10 and 15 minutes in the UK summer sun, without sunscreen several times a week is probably a safe balance between adequate vitamin D levels and any risk of skin cancer”.

Who gave the advice?

The advice comes from a group of seven British health organisations, which have issued a “consensus statement”, of their unified views.
The charities include the British Association of Dermatologists, Cancer Research UK, Diabetes UK, the Multiple Sclerosis Society, the National Heart Forum, the National Osteoporosis Society and the Primary Care Dermatology Society.

Why has the advice been issued?

Vitamin D, a vitamin necessary for health, is mainly obtained from sunlight. However, too much sun exposure has also been associated with the risk of skin cancer. Over the years, confusion may have grown from the advice issued by various organisations about the ideal levels of sunlight. The announcement from these seven organisations is aimed at clearing up some of this confusion.
A spokesperson for Cancer Research UK, said:
"This joint consensus statement brings together the latest evidence on vitamin D. In representing the unified views of many different organisations, we hope to provide some clarity around this important but controversial issue. It is encouraging that our stance agrees with that of other international organisations, such as the World Health Organisation and the US Institute of Medicine.”

How do I get vitamin D?

Most of a person’s vitamin D is made in their body through exposure to sunlight. The report says that the time required to make sufficient vitamin D varies according to a number of environmental, physical and personal factors, but is typically short and less than the amount of time needed for skin to redden and burn. Enjoying the sun safely, while taking care not to burn, can provide the benefits of vitamin D without unduly raising the risk of skin cancer.
Vitamin D supplements and specific foods can help to maintain sufficient levels of vitamin D, particularly in people at risk of deficiency. However, there is still a lot of uncertainty around what levels qualify as “optimal” or “sufficient”. There is also some uncertainty over how much sunlight different people need to achieve a given level of vitamin D, whether vitamin D protects against chronic diseases such as cancer, heart disease and diabetes, and the benefits and risks of widespread supplementation.

How much vitamin D does a person require?

How much vitamin D a person has in their body is best shown by measuring the level of 25-hydroxyvitamin D (25(OH)D) in their blood. There is a general consensus that levels below 25nmol/L (10ng/ml) are “deficient”. However, there is currently no standard definition of what the optimal level of vitamin D is, and vitamin D levels can vary between individuals.
Some scientists suggest that 70-80nmol is best, but others suggest that vitamin D levels may plateau at this level and not get any higher regardless of how much sunlight or supplements are taken. The report cites a Hawaiian study, which found that “half of healthy young surfers had levels below 75nmol/L despite extensive unprotected outdoor exposure and tanned skin”. Vitamin D can also be obtained through the diet, particularly through oily fish. However, estimates suggest that 90% of the vitamin D requirement comes from sunlight.

What about vitamin supplements?

The Department of Health currently recommends a daily 10microgram vitamin D supplement for people at risk of vitamin D deficiency, including pregnant and breastfeeding women, older people and people who may not have adequate sun exposure, such as those who are confined indoors or cover their skin for cultural reasons. Vitamin D is present in a range of dietary supplements (which are unlicensed) including fish oil products such as cod liver oil. Supplements that also contain vitamin A (including cod liver oil) are unsuitable for older people and pregnant women.
For most people however, the benefits and risks of taking vitamin D supplements are unclear. The report suggests that there is still a lack of evidence about the risks of “chronically raising” the populations levels of vitamin D through supplementation.
The report did not include figures of the estimated proportion of the UK population that is deficient in vitamin D.

How much sun should I aim to get?

The report says that ultraviolet B (UVB) radiation is the best way to boost vitamin D, but that it is unclear how much sunlight is needed to raise blood levels to a particular level. According to the report, environmental and personal factors affect vitamin D production in the skin making it difficult to make a “one size fits all” recommendation for the whole population. For example, the area of skin exposed to the sun will influence the amount of vitamin D that is made.
The report says that during the winter months in the UK, there is not enough UVB for the body to make vitamin D. Therefore, the body relies on stores of vitamin D and vitamin D obtained through the diet.
The report quotes data from a study in which caucasian British people were given a dose of simulated sunlight equivalent to midday summer sun for 13 minutes, three times a week for six weeks during the winter months. The participants were dressed in typical summer clothes that revealed a third of their skin. This raised vitamin D blood levels to more than 50nmol/L in 90% of people, and to more than 70nmol/L in 26% of people.
This study appears to be the basis for the recommendation of 10-15 minutes midday summer sun quoted by the newspapers. It is important to point out that the report did not actually specify a recommended time that people should spend in the sun again highlighting that the time required may vary dependent on clothing, the amount of shade, how much time people typically spent outside and so on. They said “regularly going outside for a matter of minutes around the middle of the day without sunscreen should be enough”.

What about sunbeds?

Sunbeds were not recommended as a way to top up vitamin D. Alongside UVB, sunbeds emit UVA which can cause skin cancer and does not contribute to vitamin D production.

Cancer Research UK has joined with the British Association of DermatologistsDiabetes UK, theMultiple Sclerosis Society, the National Heart Forum, the National Osteoporosis Society and thePrimary Care Dermatology Society to issue a joint position statement on vitamin D.
While it has a number of known benefits, there are still a number of unanswered questions associated with the vitamin.
These include what levels count as "sufficient" or "optimal" and whether vitamin D can help to protect against cancer, heart disease, diabetes, multiple sclerosis and other chronic diseases.
Furthermore, the fact that the sun's rays are people's main source of vitamin D raises questions about the trade-off between under- and overexposure, with the former linked - through vitamin D deficiency - to bone conditions such as rickets in children, and the latter to skin cancer, including melanoma.
As a result, the organisations have presented their unified views in an attempt to provide clarity.
The statement emphasises that while vitamin D is essential for good bone health, the evidence which suggests it can protect against cancer, heart disease, diabetes, multiple sclerosis and other chronic diseases is still inconclusive.
Meanwhile, it is stressed that a standard definition of an 'optimal' level of vitamin D has yet to be established, and that there is no 'one-size-fits-all' answer to the sun exposure question.
Instead, people are urged to enjoy the sun safely and take care not to burn, helping to ensure the benefits of vitamin D can be enjoyed without the risk of skin cancer being raised unnecessarily.
Finally, dietary modifications are recommended during winter time to boost vitamin D levels, although food fortification or widespread vitamin D supplementation for the general population is not advocated at this stage due to a lack of evidence about effectiveness and possible harms.

"In representing the unified views of many different organisations, we hope to provide some clarity around this important but controversial issue. "While some sunshine is good for us and vitamin D is important for good bone health, the evidence suggesting that vitamin D protects against other disease such as cancer or heart disease is inconclusive.
"By enjoying the sun safely, people can make enough vitamin D without increasing their risk of skin cancer."
